MintRx and the healthcare entities involved in your care follow applicable privacy and security requirements for protected health information (PHI).

We provide notice of relevant privacy practices and take appropriate steps if a breach may have compromised the privacy or security of your information.

PHI may be used or shared for treatment, payment, healthcare operations, and other purposes permitted or required by law.

This can include coordinating provider review, supporting pharmacy fulfillment, processing payment, operating the service, and responding to lawful requests.

Subject to applicable law, you may ask to:

• Access or receive a copy of certain records
• Correct information you believe is inaccurate
• Request limits on certain uses or disclosures
• Receive communications in a specific way
• Receive a list of certain disclosures
• Receive a paper copy of this notice

If you believe your privacy rights were violated, contact Client Care. You may also file a complaint with the U.S. Department of Health and Human Services.

Filing a privacy complaint will not affect your access to services.
